Newborns

We see babies often during the first few
years of life to monitor growth and development, review safety issues and keep
immunizations current. Our initial introduction to your baby is usually within
3-5 days of birth, with the initial examination happening 2-3 days after
discharge. Most babies need additional weight checks during the first few
weeks of life to be sure weight gain is appropriate. After the first few
weeks, we’ll have routine well checks that coincide with immunization
visits.

Well Visit Schedule

These visits occur at 3-5 days of life, 1 month, 2 months, 4 months, 6 months, 9
months, 12 months, 15 months, 18 months, 24 months, 30 months, and then
yearly.

Your Family's Medical 'Home'

We believe in the patient centered medical home model of care that emphasizes

care coordination and communication. All children deserve a medical home - a

place where their care is accessible, family-centered, continuous, comprehensive,

coordinated, compassionate, and culturally effective.

Health Forms

Schools, day-cares, scouting, athletics, and other organizations often require

health forms to be completed prior to participation. Kansas state law requires all

high school and middle school athletes to have a sports physical after May 1st and

before the sport season begins. Please plan accordingly if you have a teen in sports.

Insurance only allows one well visit per year!

Please review the links below for any forms you may need.
